GPS Credit Application

Thank you for your interests in opening a DHL account with GPS.  To set up your DHL account, we will need a copy of your credit application..

If you are opening a DHL account as one the followings:

• Corporation;
• Limited Liability Company (LLC),
• Sole propriter with a DBA.

Please download A Copy of GPS Credit Application, complete it and send it to us.

GPS also accepts application from individuals (sole propriters without DBA's), in such cases, we will ask for a copy of your photo ID (driver's license or passport) and images of the front and back of a major credit card under your name.
